Good documentation is not a library of perfect documents. It is a small set of dependable places where people can answer common questions without waiting for somebody to come online.

Start with four pages

  • A team handbook explaining how work moves
  • A decision log recording what changed and why
  • A project brief with the owner, outcome and current status
  • A short directory showing where recurring information belongs

Give every page an owner

Ownership does not mean writing everything. It means noticing when a page no longer reflects reality and making sure the right person updates it.

Document decisions, not conversations

Chat history is useful evidence but poor documentation. Capture the conclusion, alternatives considered and the date the team should revisit it. Readers rarely need the full path through the discussion.

Make maintenance part of the work

Add a documentation check to project reviews and retrospectives. A five-minute update at the end of a project costs far less than reconstructing its context six months later.
